Thoughts for completing a DAT Form

The notes you read at the bottom of the form are the result of our collective experience; we have written them by accompanying people who were faced with an unfortunate diagnosis in filling out the DAT, discovering with them how many points cannot be taken for granted, how much more information is needed, how many things we discover we do not know... This version of our form full of notes should be understood as a tool for reflection to facilitate filling in, in which we are often alone, without someone to discuss issues that are sometimes still taboo in our culture.
